What this covers.
This policy describes how Maz Ghodsian, D.O. ("we," "us," "the practice") collects, uses, and protects information you provide through this website and during your care. It is intended as a plain-English summary; the practice's full HIPAA Notice of Privacy Practices is the authoritative document and is available at any office.
Information we collect.
We collect information you actively provide — through booking forms, photo consult submissions, telehealth visits, and in-office care. This includes your name, contact information, and the medical information necessary to provide care.
We also collect limited technical information when you visit this website (browser type, pages visited, general location) for analytics and security purposes. We do not use this information to identify individual visitors.
How we use it.
Medical information is used to provide your care, coordinate with other providers when necessary, and operate the practice (scheduling, billing, follow-up). The self pay structure of the practice means we do not submit claims to insurers — your medical information is not transmitted to insurance companies for billing purposes.
We do not sell, rent, or trade your information to third parties.
How we protect it.
The practice maintains administrative, physical, and technical safeguards consistent with HIPAA requirements. Telehealth and photo consult submissions are transmitted through secure, encrypted channels. Only the clinical staff directly involved in your care has access to your records.
Your rights.
You have the right to inspect and copy your medical records, request amendments, request restrictions on disclosure, request communication through alternate means or locations, and receive an accounting of certain disclosures. Requests should be submitted in writing to the office.
Cookies & analytics.
This website uses cookies to keep the site running smoothly and to understand aggregate visitor behavior. You can disable cookies in your browser settings; the site will still function. We do not use third-party advertising trackers.
